This is a program for you or your family members who have a chronic disease such as who have diabetes, high blood pressure, heart disease, or COPD.

Learn what you can do to manage your health.

The series is offered in weekly classes – 2 1/2 hours each week for 6 weeks. There is not charge. Space is limited. Pre-registration is requested.

Topics include:

• Understanding the Disease
• Selecting healthy foods
• Planning healthy menus
• Staying active
• Dealing with stress
• Delaying complications
• Learning to communicate with your family and health care team.
• Setting goals.
• The classes will be on Tuesday mornings: June 11 to July 16, 9:30 a.m. to 12:00 noon.
