Chronic Disease Self-Management Program

Chronic Disease Self-Management classes will be offered beginning July 8 and will meet each Monday through August 12, 2019. The classes will meet from 10:00 a.m. to 12:30 p.m. at the Parker County Extension Office, located at 604 North Main Street in Weatherford. The classes are free of charge.

The Chronic Disease Self-Management Program is a nationally recognized program that was developed at Stanford University. The 6-weeks evidence-based program helps people living with long-term health concerns and chronic diseases such as diabetes, heart disease, long-term pain, breathing problems, depression, arthritis and more.

The class will cover topics on managing symptoms, healthy eating, medication usage, stress management, making informed treatment decisions, and working with your health care provider.

There are three ways you can sign up for the class, one is by calling the Parker County Extension Office at 817-598-6168, calling Texas Health Resources at 1-877-847-9355 or go online to TexasHealth.org/Classes.
